Galaxy NoteAlso had the same issue but eventually received my voucher the afternoon after sending through my stuff to the email provided, and subsequently received my Buds Live two days later.
please try that email if you haven't already

Galaxy NoteAfter I spoke to call center people they asked me to email and I have email to the following email id
1.Verify@samsung.com
2.Tellus@samsung.com
3.Zaestore@samsung.com
To which only the 3rd email gave me a response that they are escalating the problem to concern team members and nothing has happened till now.
