SD Card Problem

i have samsung galaxy j5 2016 (J510FN)
My SD Card Automatically Removed Any Time..
Please Solve My Problem...

insert sd card in another device to check sd card is in good condition or it corrupt

settings-> Device maintenance-> Storage-> option (right up) ->storage settings-> click sd card (unmount SD card)......
