Noticed recently when cleaning my phone and case there's a gap between the screen and frame for the ear speaker... I assume this is by design and doesn't affect the IP68 rating? Seems strange there is no grill here to keep out dust and fluff etc. Is it normal or its only in my phone?

I too have it in my phone . I just bought the phone few days back . I did notice it when I got the phone which I asked to the shop who sold me they didn't know anything about it . They just sell it right. It's upon samsung to keep quality check to high levels . I'm gonna test it in water if it's a faulty device will send it to samsung warranty
