Once clear data of home screen then check if it fixes

Already tried but that too is temporary solution.
after clearing data, when i install or uninstall any app, again is looks like that!!

Then it might be software glitch.. Use smartswitch pc to update over ota next time or visit service center for reflashing firmware.. It will resolve any issues with system.
