I was in a call while changing the call background, and then I noticed that the background changed for the other contacts, but not for the person I was talking to while changing. The old background appeared for that contact.
Turn off secure folder, and tap and hold the notification. There's a slider to turn the notification off. After that, this is gone. Had the same problem.
