It seems that you are just a novice in using your phone. So, either backup important stuff and factory reset the phone or uninstall all third party apps and install only the apps necessary, one by one, while checking for unusual behaviour.

When u saw add again then just press multitasking button and see which apps doing this

i was geeting same issue.."so press multitasking button " and now you are able to see..which app are causing this issue..

lol uninstall all untrusted apps... u have apps like cymera.. dictionary etc.. they are not trusted... u have premium phone thn be a premium user... just unistall all third party apps.. and choose only trusted apps like google translate etc.. jugnu nyka modren kr ke kuchh hai ye sab faltu apps h
