binary update at service center contact them I was also facing same issue

all above your phone are in safe mode then good result from proximity sensor

first your phone are switch off, and switch on when appear sumsung press decrease volume button in 7 seconds, your phone are in safe mode ,and restat ur phone , now good result from proximity sensor
