Question, help

I want to give my tab a 10.1 to service centre becoz its screen come outside like it's gum is loss in some areas, so i want to bring it to service centre, so now what I have to do for that?

Any one suggest me which steps I have to follow from my side?

And any idea about it's cost? (Screen is properly good)
