Battery Issue

I just Checked and found out that my battery health is in Bad condition. And believe me its in bad condition. The charging speed is low and it's consuming lots of battery and IDK where it goes. 

Any solution on this?

Is it true that if they open ur phone then phone will start giving issues eventually
