What it means is, when you click clean now, all the apps that are currently running in the background, get cleared from memory. So if u still need to clean ur ram but u don't want a specific app to get removed, for example: Instagram.
Then u click exclude instagram and you can clean the ram and it will remain in the ram and everything else will be cleared. Hope this helped you.
