Should it be digitally signed? or just take a printout and sign and manually send them!!

I signed it digitally on word. Won't matter as such i feel as it is only an acknowledgment.

Take a print out then sign it manually, scan it and then send it back

You have to fill all the fields of excel sheet too and then you have to send both file in reply of that mail.
