Galaxy SFirst go to Get Help tab on this app and select Diagnostic. Then select USB connection. Run the test and check whether it's working normally. (As in the Screenshot)
If you dont get message like in the Screenshot, Try to use a different OTG adapter and a pen drive.
Still you have the issue, rest to your device. But still it's not working, it could be a hardware issue. Then you have to contact a Samsung service center near to you.
