Galaxy SThis is exactly what is happening on my end as well,
for having the S line flagship phones this is a very disappointing issue.
This seemingly interferes with music playback via wireless earbuds also where you can't control the music anymore such as play/pauze , skip and previous.

Galaxy SThis happened to me too!! Since yesterday my alarm did not go off and then would not play music on my headphones. Thisbmorning I realized what it was when I tried raising the volume of my alarm and it said it could not play sound due to being in a phone which I wasn't. Restarting the phone did solve the issue but am afraid it could happen again. Samsung please fix this!!!

Galaxy SI'm having the same issue. It has happened twice when getting into my car, I notice my Pandora app won't play after hitting the play button. Then I look at my interior dashboard and it says I'm on a call, which I'm not. The first time it happened was when I had gotten off the phone (not in the car) with my husband. The 2nd time was when I rejected a face time call from the Facebook messenger app. The only way out of it was to "hang up" by the end call button on my steering wheel. This morning it happened when my alarm only made a beep sound, and when I went to change my alarm settings that I never changed, it said I couldn't do it while on a call. I had to restart my phone.

Galaxy SI'm having the exact same issue with my s22 and I'm worried every night about my alarm since I was late for work the first time this happened and the alarm didn't go off. Hoping samsung or someone will have a better solution than just restarting my phone every night.

Galaxy SI have the same issue with the Z Fold 5. My guess it is an android issue but not 100% sure.
Android version 14.
I closed all apps and turn off Bluetooth and WiFI but still same issue.
Phone says I am still on a call and I don't even use my phone to make calls. People try calling me but I don't answer the phone. Issue seems to occur only when you don't answer the inbound call. Error occurs all day until I reboot the phone. So I have no idea on what app setting is causing this issue.

Galaxy SI am having the same issue and the only other way I have been able to temporarily "resolve" this is to hang up the call from my automobile repeatedly. If i just hit hang up once, it reconnects with no sound. This is a fairly annoying and inconvenient bug.

Galaxy SI have a Samsung Note 9, so older but still a flagship. I have been having this problem also. It was at least 9 months ago that I noticed. At first I thought it was a problem with my earbuds because I had very low volume. Occasionally my alarm wouldn't ring, but I would hear the phone buzz because it was vibrating on my nightstand. After I got a new pair of earbuds (8 most ago) and the problem continued, I discovered that anytime I was experiencing the problem my volume settings would show "in call volume" as one of the sliders even though I was not on a call. I have not found any thing that consistently triggers it or remedies it. This thread is the first thing that I have found that is actually addressing the same problem that I am having. None of the solutions offered here have helped. I have the most current version of both the Google clock and the Samsung clock apps installed. I have also found that in sometimes when I go to play a voicemail, nothing happens. When I tap okay a second time, it says "please wait audio controls resetting". After that, the voicemail will play. Sometimes that seems to fix the stuck on in call volume problem, but definitely not always. This problem does not affect volume if I plug in wired speakers or earbuds. I also experience my phone randomly turning the notification volume or media volume all the way down on its own. Sometimes the media volume suddenly goes to zero while I have music or video playing. Since this also has to do with volume control, I suspect that it is part of the same issue.
