Galaxy SAfter that reset your Galaxy wearable app. (Settings > Apps > Galaxy wearable > Storage > Clear Cache > Clear Data). Also make sure that, it is away from other paired devices.

Galaxy SThen restart your phone.
Also please make sure that, it's not getting interfered by your wifi router. Check with a different location. If that don't help, please explain your issue in detail.
