Have you tried resetting the device?

Try clearing storage from app info of camera app. If it doesn't help, try a reset after creating a backup..

Looks like I don't have a choice.. I have to reset the phone

@MittuVichu: luckily I was able to fix it with out the factory reset. It was not working because camera access was disabled in quick access panel. It took time for me to figure it out. Once I anble and disable it again, now the camera app is asking for permission unlike the previous case.
