Maybe sometimes it happens when auto focus turned off.

Take a photo in high resolution and zoom after saved to the gallery. Then zoom while taking. See the difference between them. Send screenshots if you can

I read this back on Reddit, users saying the same issue. I think it is their on the Note 20
Due to aperture issues and light focus problems

Other than that if its an serious issue try,
• Washing the camera lens with the help of a slightly damp cotton swab.

• Checking Resolution by going to your camera app > Settings and then click Resolution.

• Checking camera AF in Auto Focus mode.

• Try through a 3rd party camera app from the play store. (I use Photoshop Lightroom Camera and the best : GCam Apk from the web).

• Updating Software

• Force stopping the Camera app through settings > Apps > Camera > Force stop.

• Resetting the App Preferences through settings > apps > Three dots > Reset app preferences.

• Reset your Device.

• Go to Service Center.
