When I charge my phone to 100% and unplug the charger, the battery charge immediately comes to 96% without any use. How to fix it. Should battery be replaced ?

It seems the battery capacity is reduced. Don't change your device upto 100%, and don't drain it to 0%. Use between 20% - 95%. When the charge drains near around 20% then charge it to around 90%.
